The Underwriting Process
In Utah, an insurance comapany will offer three different outcomes to a submitted application:
Your application will be approved.
Your application will be approved, but you monthly premium will be increased

to cover risk due to your medical history or current or past medications. You

will be made an offer for the plan you applied for, at a higher price.
Your application will be denied, based on eligibility or medical history.
Health Requirements
In order to qualify for a individual or family health plan, you must meet the following requirements:
You must be relatively healthy (this definition varies for each company)
Any current medical conditions must be under control
All pending surgeries and/or treatments must be completed

Residency Requirements
In general, you must reside in the state of Utah at least six months out of each year to qualify for an individual or family health insurance plan in the state of Utah. Some companies require you to be in the state 100% of the time, with the exception of vacationing.
Altius and SelectHealth require that you do not attend school, work, or reside outside of the state of Utah at any time during the year. HumanaOne and Regence Blue Cross Blue Shield of Utah are more lenient and allow you to be out of the state for one of these reasons for up to six months per year.
